Subject: Complaint against Care Health Insurance for unfair claim rejection due to lack of proper disclosure during policy porting
Respected Sir/Madam,
I am writing this complaint against Care Health Insurance regarding the rejection of my insurance claim, which I believe is unjust, arbitrary, and a direct result of improper policy porting procedures followed by the insurer.

At the time of porting my health insurance policy to Care Health Insurance, no representative of the company explained to me the advantages, disadvantages, waiting periods, exclusions, or the impact of porting on future claims. I was never educated about the policy terms in a manner that I could clearly understand.

As a customer, I was not informed that signing a declaration meant I had fully understood all conditions related to policy porting. No official from Care Health Insurance visited me or contacted me to ensure that I was properly educated about the policy. The declaration was taken without proper explanation, which defeats the very purpose of informed consent.

It is the duty of the insurance company’s sales and servicing staff to clearly educate the customer before policy porting and to obtain confirmation only after ensuring that the customer has fully understood all implications. In my case, this responsibility was completely neglected.

Due to this failure on the part of Care Health Insurance, my claim has now been rejected, causing me mental stress, financial loss, and hardship. This amounts to deficiency in service and unfair trade practice under consumer protection laws.
Hence, I request the Hon’ble Authority to:

  1. Review the claim rejection made by Care Health Insurance.
  2. Direct the insurer to settle my claim fairly.
  3. Take necessary action against the insurer for failing to follow proper customer education and disclosure norms during policy porting.
I am submitting this complaint through the Insurance Ombudsman / Consumer Court route to seek justice and appropriate relief.
